New to Okotoks and need to transfer your player into OMHA
If you have recently moved to Okotoks from within Alberta and your player has played sanctioned hockey before, please complete the below player movement form.
- Please complete the form including the date. Once you have filled out the top half of the page please email it to your old Minor Hockey Association. Once they fill out the Primary MHA section please email lindsaygraw@shaw.ca the form.
- Once Lindsay receives the completed form she will initiate the transfer.
If you have recently moved to Okotoks from outside Alberta and your player has played sanctioned hockey before, please complete the below Parent Declaration form.
- When asked to enter an email address for the MHA for which you are registering please enter: lindsaygraw@shaw.ca
- Please upload 3 proof of residency documents from the list below.
Examples of residency documents are:
Driver’s License for Father
Driver’s License for Mother
School registration at a school in or near the residence for the current year
Proof of mail delivery by Canada Post at residence (Box numbers are not acceptable)
Telephone bill for land line service at residence
Cable or Internet bill for service at residence
Utility bill for service at residence
Lease agreement for residence in parent(s) name(s)
Current property tax notice in parent(s) name(s)
Homeowner/Renter’s insurance policy for residence
Void cheque with residence address
Vehicle registration with residence address.
Any other documentation which supports your claim to residency at the address
Once the Parent Declaration form has been submitted please email Lindsay Graw for confirmation of receipt. Once Lindsay has the completed form she will initiate the players transfer into Okotoks.
Players Hockey Canada profiles MUST be in Okotoks prior to attending any skates.
